Winner Social Housing Adaptation Service of the Year 2021
WDH is a Registered Social Landlord, transferring from the Local Authority in 2005. WDH provides accommodation for over one third of the Wakefield Districts population and goes beyond just contributing to the delivery of adaptions by committing £1m per year to do so, including major works up to £25k.
Recognised as an enabler to meeting health and social care partner objectives, the service WDH provides is fully integrated with the ‘Connecting Care’ agenda in Wakefield. Their OTs can effectively collaborate with other therapy teams to ensure the appropriate service leads on a patients care. WDH now offer wrap around support via Wellbeing Caseworkers, Mental Health Navigators, their Adaptations Service having grown to encompass wellbeing services and working with commissioners at Wakefield CCG. They also have Housing Coordinators working the hospital wards to effect timely discharge and prevent any bed blocking. For tenants who have needs that can’t be met by adaptations alone, WDH offer a wealth of support and assistive technology.
